Service Description:

10-week program for people who have Chronic Obstructive Pulmonary Disease (COPD), including those recently hospitalized due to COPD * services provided by a certified registered educator, kinesiologist, physiotherapists, occupational therapist, dietitian, social worker, and psychologist

- diabetes education, exercise program, and kitchen
Progression level of rehabilitative care for patients working to restore or maximize functionality
