Point of access to mental health intensive case management services provided by the Canadian Mental Health Association - Hamilton Branch, City of Hamilton Mental Health/Outreach Team (Community Mental Health Promotion Program), Hamilton Program for Schizophrenia, and Hamilton Mental Health Outreach

Eligibility / Target Population:
|
Hamilton residents diagnosed with a serious and persistent mental illness (excluding personality disorders and mental retardation)* must be under the care of a physician * must be willing and able to participate by working on goals, and requires support to maintain wellness in the community
