Service Description:

Assessment of patients with low back pain and/or low back related leg pain * services include personal self-management plan to help patients manage their low back pain * patients are assessed by a chiropractor

Referral required from a primary care provider or nurse practitioner who are enrolled in the program

Eligibility / Target Population:
|
Adults 18 years and older who have unmanageable low back pain and/or related symptoms that have lasted for at least 6 weeks but not longer than 12 months, and those whose pain and/or related symptoms occur some of the time, with less than 12 months between occurrences
