Service Description:
|
Non-emergency community-based health care in which paramedics provide care in partnership with primary care, healthcare teams, and other community services * provides preventative, at-home health care visits for citizens who meet the eligibility criteria * provides wellness clinics in identified large multi-unit dwellings * focuses on improving health outcomes and quality of daily living, reducing avoidable use of emergency department and 911 services * helps vulnerable and at-risk people live safely in their homes * services include:
- health and medical risk assessments
- education
- wellness clinics
- chronic disease management
- medication management
- influenza (flu) prevention and immunization
- various point of care tests
- blood-work for program clients unable to get to a lab
- urinalysis and urine cultures for program clients unable to get to a lab
- administration of various medications
- diabetic sensor changes
- COVID-19 support for fall surge
- remote patient monitoring
- fall risk assessment and support
- mental health and addictions support
- homelessness and under-housed support
- social isolation support
- school and group education
