                            | Service Description: | The goal of the RCM is to ensure vulnerable populations receive appropriate clinical support and monitoring in the community, including escalation to medical practitioners or acute care, where necessary * helps to reduce unplanned visits to the Emergency Department and/or unplanned re-admissions and develop sustainable self-management techniques for the identified patient populations supported through education and resources * allows patients with acute viral respiratory illnesses, Chronic Obstructive Pulmonary Disease (COPD), Congestive Heart Failure (CHF), palliative (prognosis 2 to 12 months), to manage their symptoms at home with technology and nursing support provided to them 
 
 The program is a partnership between Halton Healthcare (HH), Connected Care Halton Ontario Health Team (CCHOHT) and Ontario Health atHome * patient vital signs are reviewed by a health care provider remotely
