Eligibility / Target Population:
|
ED and/or inpatients designated ALC, or at risk of being ALC and requires at least 1 professional service, and has optimization, restorative, reactivation, or rehabilitation needs at least one professional service and can include PSW, therapeutics team members, navigators/coordinator available
