                            | Service Description: | Provides outpatient care, education and support to children and adults with type 1 and 2 diabetes, prediabetes and gestational diabetes * team consists of diabetes educators (nurse educators and dietitian educators), social worker, kinesiologist (exercise specialist), endocrinologists, nurse practitioner, and support staff * supports patients learning knowledge and skill on diabetes management and staying on track with healthy living behaviours * education is provided through individual appointments, group programs, and workshops at a variety of locations in the Halton region
