Service Description:
|
Hospital-based outpatient rehabilitation for adults who have had a stroke, acquired brain injury, spinal cord injury, or have complex musculoskeletal conditions * provides assessment and time-limited treatment to:
- maximize recovery from a recent acute event
- maximize functional abilities and patient safety
- promote adaption of/to home
- support transition to appropriate community-based programing
- support timely transition from acute care to inpatient rehab setting
- prevent admission or re-admission to an acute or rehab hospital
- provide opportunity to learn/practice in a familiar, stimulating, and supportive environment
- provide goal-driven treatment based on measurable progress and functional change
