Community-based supports for people at risk for or affected by HIV, Hepatitis C, and sexually-transmitted and blood-borne infections * services include:
- case management
- individual and couples counselling
- social and educational events
Home Support Program - Empowers individuals living with HIV to maintain their health, independence, and quality of life through tailored services- assessment and individualized care plans
- in-home support for activities of daily living (ADL's)
- medication management
- accompaniment and support during medical appointments
